Required Documents

To ensure your child is fully enrolled and compliant with state requirements, all required enrollment documents must be submitted to the school office. Please note that health forms must be completed on approved Kentucky forms, regardless of whether your child receives care in Kentucky or Tennessee.

Preschool Students

  • Birth Certificate
  • Social Security Card
  • Up-to-Date Kentucky Immunization Certificate
  • Kentucky School Physical Examination Form (for the current school year)

Kindergarten-5th Grade Students

  • Birth Certificate
  • Social Security Card
  • Up-to-Date Kentucky Immunization Certificate
  • Kentucky School Physical Examination Form (for the current school year)
  • Kentucky Dental Screening Form
  • Kentucky Vision Examination Form (completed by an optometrist or ophthalmologist)
